Complete the Membership Application
Membership Eligibility
Membership will be open to all nurse practitioners. Members will be asked to complete five areas on the membership application: 1) area of NP specialty certification; 2) area of current practice; 3) regulation by state licensure to practice with a supervising physician or as a licensed independent practitioner (LIP); 4) the specialty of the supervising physician; and 5) whether you have prescriptive authority. Members will be expected to update this information with the DNA, as it changes.
